Do not submit protected health information
Our website forms are ordinary, unsecured web forms and are not intended for protected health information (PHI). Do not submit patient names, dates of birth, medical record numbers, diagnoses, test results, or any other patient-identifying information through this website. If a shipment requires that information to be coordinated, call us at (360) 383-5945 so it can be handled through an appropriate channel.
Information we collect
We collect the information you choose to provide through our pickup request, quote request, and contact forms — typically your name, organization, email address, telephone number, pickup and delivery addresses, and the service details you describe. We also record the date and time of your submission and a one-way hashed form of your network address, which we use only to limit abuse and duplicate submissions. We do not store your raw IP address with your submission.
How we use information
We use submitted information to respond to your inquiry, evaluate and schedule courier service, communicate with you about a request, and maintain business records of the services we perform. We do not sell your information, and we do not use it for advertising.
Sharing
We share information only with personnel and service providers who need it to operate this website or perform the requested service (for example, our website hosting and database provider), and where required by law or to protect our legal rights. Service providers are permitted to use the information only to provide services to us.
Protected health information
Where HolmesLabs LLC performs services for a covered entity and encounters protected health information in the course of transportation, that information is handled in accordance with applicable law and the terms of the written agreement between TyVia and your organization, including a business associate agreement where one is required. Website form submissions are not covered by any such agreement.
Security and retention
We use reasonable administrative and technical safeguards to protect the information we hold, including access controls on our request records. No method of transmission over the internet is completely secure. We retain request and contact records for as long as needed for the purposes described above and to satisfy legal, tax, and recordkeeping obligations.
Cookies and analytics
This website uses only the storage necessary for the site to function, such as maintaining a session for authorized administrative users. We do not use advertising or cross-site tracking cookies.
Children
This website is intended for business use and is not directed to children. We do not knowingly collect information from children.
